Fillable Forms: To use the fillable forms provided, download a copy, save it to your hard drive, complete the form, save your changes, print or submit the completed form digitally. Fillable forms can also be printed and completed by hand.


1. Permit Application, Initial Submittal Requirement (ISR) sheets, Checklists: Before filling out your permit application, download the appropriate ISR or Checklist below to ensure you have all the required documents for submittal. 

2. Initial Submittal Requirements (ISRs): ISRs are designed to guide you and they provide important information about the submittal requirements for specific permit types. They help ensure all required documents are submitted with your permit application. Each ISR links directly to the documents needed for submittal. Additional information still be required by the Building Plan Review and Inspection Division during plan review.

3. House Pad Requirements:
This form must accompany any permit (address) adding a structure (pool, shed, etc.) that does not require a Stormwater Management Permit, (any zoning that is not in a PUD, or does not have a South Florida Water Management Permit).
